My initial question is appended at the bottom.
Here is what I now know about OSF-BASE

The OSF-BASE license does not come as software. An OSF-BASE is a piece of
paper with the information one needs to enter it via lmfsetup.
The information comes in a format that is used by lmfsetup
do the following:

o- Find your piece of paper or call compaq at 1-800-DIGITAL 
   (I have to warn you they threw me around for a loop and 
    got no answers. The term "osf base" sounded like a foreing
    language to them.)
o- cp /usr/var/adm/lmf/template /usr/var/adm/lmf/osfbase
o- vi /usr/var/adm/lmf/osfbase
   (Here append the info to the corresponding fields matching
    your OSF-BASE hard copy)
o- /usr/sbin/lmfsetup /usr/var/adm/lmf/osfbase
o- Just in case reset your licenses: lmf reset
You may also enter the osf-base info interactively by issuing the command
alone.
