sorry, for late summary. Any way, I received on responded from
Oisin Mcquinness, he said to stop & start LSM and did that and it works.

     Hi everyone,
     
     I have a system alpha 4100, running DEC UNIX V4.0D with 
     patch kit #3.   LSM was up running fine before, at least
     up to this week.  Now when issue the LSM commands as you can 
     see below and it doesn't seems to work.   What could be wrong? 
     Can anyone please tell me...?
