AlphaServer console failed to send read?

The Question is:

 
I'm having problems booting the server. The message says "failed to send
read to dkc400" where dkc400 is the boot disk.
 
Please help me!

The Answer is :

 
  First, ensure that you have the current SRM console firmware installed.
 
  There is usually other information included with the the SRM console
  error "failed to send read to...".  Also ensure that the setting of
  the BOOTDEF_DEV environment variable is correct.
 
  Then ensure that you have a valid system disk and system root available
  and no disk errors or bad blocks are involved -- consider using the
  CD-ROM bootable environment to mount and verify the system disk.
 
  Please contact the Compaq Customer Support Center for assistance.
  (Expect to be asked details of your specific Alpha system, the system
