SCSI Disk Offline?

The Question is:

 
During the install the HD DKA400: is shown as offline. It is also shown as
 offline with an error count when DCL comands are loaded.
 
The machine is a 500au / B3AAA-R3
The Drive is a Compaqpc Atlas IV 18 Gbyte.
Firm ware Update V5.1
OS OpenVMS V7.1=1H2
Install boot =b -fl 0,80000 dqa0,dva0

  error, a SCSI termination error, a controller error, a SCSI signal
