KZPBA-CY
The DEC KZPBA-CY (option KZPBA-CB) is a PCI High Voltage Differential UltraSCSI Host Bus Adapter. DEC seems to have sold these primarily for Alpha workstations and servers, but the manual suggests they should be compatible with x86 PCs as well. The KZPBA was manufactured by QLogic for DEC/Compaq and appears to be based on the QLA1041, though unlike the KZPBA some reports on Usenet suggest the QLA1041 is not compatible with DEC Alpha systems. DEC sold the non-differential variant of this card as the KZPBA-CX (option KZPBA-CA).

Certain Alpha systems with AlphaBIOS 5.66 or newer support running the adapters x86 SCSI BIOS (enabled from the Advanced CMOS Settings Menu), allowing the cards embedded configuration utility to be used instead.

Windows NT 4.0 has a suitable driver on the CD-ROM, so this shouldn't be required.
